Instructions for Ordering FormDocs CMS-1500

Thank you for your interest in purchasing FormDocs.

Please note that FormDocs has temporarily suspended its on-line credit card processing feature with world-wide provider Digital River due to problems that credit-card processor is experiencing. In the interim, FormDocs will be accepting payment for its products and services solely by the alternate means described below.

USA & CANADA INDIVIDUALS

Please send a check or money order postal mailed to our company address below. Although we recognize that this represents an inconvenince, we are confident that it is the safest and most efficient method of ensuring the integrity and validity of your FormDocs product and license information. Orders will be processed Monday through Friday from 8AM to 7PM ET. FormDocs software download link(s) and license information will be immediately delivered to you by eMail, sent to the eMail address you provide.

1. Checks should be made out to "FormDocs LLC" in US dollars.
2. Checks must be drawn on a bank having a presence in the United States.
3. Include a printout of your order page (press Back to return to the order page, then right-click on the page and choose "Print").

USA & CANADA COMMERCIAL, LAW ENFORCEMENT, and GOVERNMENT CUSTOMERS:

We are temporarily granting credit terms NET 30 to ALL USA Commercial, Law Enforcement, and Government customers. Place your orders via a purchase order e-mailed to "sales@formdocs.com". Please ensure you meet our requirements for accepting a purchase order which are the following:

1. Your Commercial organization must be registered in your headquarters State.
2. Must be on official letterhead displaying the name and address of your organization.
3. Must be "legibly" dated and numbered.
4. Must display in "printed form" the name and title of the person authorizing the purchase.
5. Must provide a phone number and eMail address for the person authorizing the purchase.
6. Must be "legibly" signed by the person authorizing the purchase.
7. Must list FormDocs descriptions, quantities, and prices as displayed on the FormDocs website 'Buy Now" page.

Upon receipt and validation of your purchase order, FormDocs software download link(s) and license information will be immediately delivered to you by eMail, sent to the eMail address you provide.

INTERNATIONAL (NON-USA) CUSTOMERS:

Please send a request for price quotation to "sales@formdocs.com", listing the FormDocs products and services you want to purchase. We will reply with Electronic Funds Transfer (EFT) Wire instructions. Upon receipt of your wire payment, FormDocs software download link(s) and license information will be immediately delivered to you by eMail, sent to the eMail address you provide.
